- Available to members under age 60
- Only 5 medical questions
- If you are under age 50: up to $250,000 of coverage
- If you are age 50-59: up to $150,000 of coverage

- Available to members under age 75
- Members choose any amount up to $1.5 million in $5,000 increments
- Spouses can choose any amount (as long as it does not exceed member coverage) in $50,000 increments up to $500,000
